Mount with Daemon Tools.  

Install.

Do not run the DX addon.

Do not run the game, after it's done.

Extract the 1.Vivi_DEU_Uncut_11.zip contents
to wherever the game is installed.

Run 2.VivisectorGoreFinal.exe

Then extract 3.VV2.rar  to the game folder.

This will:

Patch to 1.1
(fixing horrible horrible sound problems)

Add a load of 'gore'

change it to 'english' text

Then play.

I noted a 'small' problem in stage one, where you're near a gate.  Just quicksave before each 'point' to be on the safe side, up to level 2 or so, no more problems.
